    What You'll Do   

    As a Family Support Coordinator, you will be the ultimate bridge to vital resources, community connection, and advocacy:  

    Advocate & Empower: Connect families to state and community resources while maximizing their control over their own services.  

Guide Transitions: Help individuals smoothly plan and navigate the massive milestone of transitioning from school to adult services.  

Navigate Legal & Financial Pathways: Assist with budget administration and facilitate important processes like Guardianship and Power of Attorney.  

Collaborate & Document: Facilitate team planning meetings and maintain meticulous documentation alongside a passionate, friendly team.  

What We’re Looking For   

We need a flexible, creative, and non-judgmental professional who genuinely loves serving diverse families.  

  • Education: A Bachelor’s degree is highly preferred, but extensive case management experience will absolutely be considered instead.  

  • Experience: At least 2 years in human services, with a minimum of 1 year in family supportExperience with developmental disabilities is a huge plus!  
  •  
  • Skills: Excellent communication, strong organization, intermediate MS Office proficiency, and a willingness to learn new programs.  
  •  
  • Travel: Reliable transportation for regional/statewide home visits and meetings.

Competitive Pay: $22.00 to $25.00 per hour, depending on experience.
